Same-day implants are well-established in carefully selected cases. The key word is 'selected.' Not every patient qualifies, and rushing the timeline in the wrong case compromises long-term success. Same-day implants work best for patients who have a single failing tooth (or a few teeth) being replaced, generally healthy gums and bone in the area, no active infection at the planned implant site, and a bite that won't put excessive force on the temporary tooth during healing. The location of the tooth also matters: front teeth are often good candidates for the cosmetic and functional benefits of same-day treatment, while back molars under heavy chewing forces sometimes do better with a traditional timeline.
Patients who typically don't qualify include patients with active infection at the tooth being extracted (infection must be resolved before the implant is placed), patients with significant bone loss that requires grafting first, patients with conditions that affect healing (uncontrolled diabetes, certain medications, heavy smoking), and patients with bite forces or grinding patterns that would overload a healing implant.
The honest answer for most patients is: we won't know whether you're a candidate until the consultation. The 3D CBCT scan and clinical exam tell us with precision whether same-day treatment is the right call for your specific tooth and your specific situation.
The most obvious benefit is the timeline. A traditional implant case spans 3 to 6 months across multiple visits. A same-day case compresses extraction, implant placement, and temporary tooth attachment into one visit. You leave with a tooth in the gap rather than an empty space.
Bone preservation is a clinical benefit. When a tooth is extracted, the bone in that area begins shrinking immediately. Placing the implant in the fresh socket the same day maintains the natural bone architecture and reduces the need for grafting later.
For front teeth, the cosmetic benefit is significant. You don't walk around with a visible gap or a removable temporary during the healing period. The temporary tooth on the implant looks and functions well, allowing you to smile and eat without obvious change.

The total treatment timeline for a same-day implant case is similar to a standard implant case (3 to 6 months to final crown), but the front of the timeline is compressed.
Stage 1: consultation and detailed planning. 3D CBCT imaging, clinical examination, review of medical history, and assessment of bone and gum health at the planned implant site. We confirm same-day treatment is appropriate and design the temporary tooth in advance. You receive a complete written plan, timeline, and cost estimate.
Stage 2: surgery day. The failing tooth is extracted with care to preserve the socket and surrounding bone. The implant is placed immediately into the prepared socket using a computer-generated surgical guide for accuracy. A small bone graft is placed around the implant if needed to fill gaps. A temporary tooth is then attached, typically a fixed (non-removable) temporary crown. You leave with a tooth in place. The visit usually takes 90 to 120 minutes depending on the tooth.
Stage 3: healing. Over the next 3 to 4 months, the implant integrates with the bone. The temporary tooth stays in place, and you follow specific instructions to avoid heavy biting on it during this period.
Stage 4: final crown. Once integration is complete, we take digital impressions and our in-house lab fabricates the final porcelain crown. The temporary is removed and the permanent crown is attached in a short follow-up visit.
In an immediate-load same-day case, yes. A temporary tooth is attached to the implant at the same visit. In some cases (particularly where bone is being grafted at the same time, or where the bite would overload the healing implant), we recommend a temporary that doesn't touch during chewing, or a removable temporary. We'll be clear about which approach fits your case.
Once fully healed, yes. The final outcome is the same: a fully integrated implant with a permanent crown. The difference is in the front of the timeline. The trade-off is that the implant has to be more carefully protected from heavy forces during the first few months while it's integrating, so we'll give you specific instructions about what to eat and how to chew during healing.
Sometimes a small graft is placed around the implant at the same visit, particularly when there's a gap between the implant and the socket walls. This is well-tolerated and adds little to the overall recovery. Larger grafts that require separate healing time typically rule out a single-visit approach.
Active infection at the site of the implant is generally a reason to use a traditional timeline. The tooth is extracted, the infection is allowed to resolve, and the implant is placed later. Skipping this step risks implant failure. We screen for infection at the consultation and during the procedure itself.
